Dosing / renal protocol and limits

For adult meningitis, osteomyelitis, bloodstream infection or endovascular infection, the source uses 2 g IV every 4 hours. It does not reduce this regimen for renal dysfunction or hemodialysis. This is the listed serious-infection schedule, not a blanket dose for all settings.
