Dosing / renal protocol and limits

When CYP2C9/VKORC1 genotypes are not known, the label describes an initial 2-5 mg orally once daily and typical maintenance of 2-10 mg daily. These are orientation ranges, not a fixed prescription: select the dose by indication, INR response, interacting drugs and patient factors.

The label does not mandate an automatic renal reduction. Patients with renal impairment should have more frequent INR monitoring. Serious or fatal bleeding is the boxed warning; anticoagulant-related nephropathy is also relevant. A stable renal classification does not establish a stable warfarin dose.
