Dosing / renal protocol and limits

Adults: IV loading for CABP or ABSSSI is 200 mg over 60 minutes on day 1, or 100 mg over 30 minutes twice on day 1. Maintenance is 100 mg IV over 30 minutes daily or 300 mg orally daily, for 7-14 days.

Oral-only loading differs: CABP uses 300 mg twice on day 1; ABSSSI uses 450 mg daily on days 1 and 2. Do not exchange these loading schedules.

No adjustment is required for renal impairment, including hemodialysis, or hepatic impairment.

For oral doses, fast at least 4 hours beforehand; afterward avoid food/drinks except water for 2 hours, and dairy, antacids and multivitamins for 4 hours.
