PEMAZYRE: amount and cycle are separate decisions
The severe nondialysis reduction retains the indication-specific schedule. The intermittent-HD statement is a separate population, not a general ESRD instruction.
What to reconcile: Keep the continuous or intermittent cycle attached to its indication; do not extrapolate the HD statement to PD or CRRT.

Renal dosing, restrictions and evidence limits
At MDRD eGFR 15-29 mL/min/1.73 m2, the official dose is 9 mg using the schedule for the indication. Cholangiocarcinoma uses once-daily treatment for 14 days followed by 7 days off; myeloid/lymphoid neoplasms with FGFR1 rearrangement use continuous once-daily dosing. A renal dose reduction changes the amount, not this distinction between cycles. Interaction and toxicity modifications remain separate.
