Nondialysis percentage differs below CrCl 10
The RX table retains 50% of the indication dose at CrCl <=50 without dialysis. UCSF separates CrCl <10 and uses 25%. Its continuous-dialysis guidance is specifically qualified by CVVHD evidence.
What to reconcile: Select one applicable complete regimen. Do not combine a label percentage with a different institutional dialysis interval.

Renal dosing, restrictions and evidence limits
No renal adjustment is needed for the single-dose vaginal-candidiasis regimen. For multiple doses, give the indication-appropriate loading dose; the selected section describes 50-400 mg. Subsequent daily dose: CrCl >50, 100%; CrCl <=50 without dialysis, 50%. Hemodialysis: 100% of the recommended dose after each session; on nondialysis days reduce according to CrCl. Clinical condition can require further adjustment. Do not substitute this table for a continuous-dialysis protocol.
