The handbook's GFR-based 2.5 mg twice-daily AF rows differ from the U.S. label's combined age, weight and serum-creatinine criteria. Dialysis evidence is limited. No combined algorithm has been generated.
The book lists usual dosing with caution at GFR 30-50 mL/min. At 15-30 it retains usual dosing for non-AF indications but lists 2.5 mg twice daily for AF.
