Dosing / renal protocol and limits

The institutional IV schedule is 200 mg once followed by 100 mg daily. The listed oral schedule is 450 mg daily for two doses followed by 300 mg daily. The source does not reduce these regimens for renal dysfunction or HD.

Oral administration requires at least 4 hours fasting before and 2 hours after a dose. Loading may be omitted for selected NTM regimens under specialist care.
