Label guidance versus selected expert dose reduction
The label does not require routine renal reduction. UCSF describes a selected strategy for stable patients with CrCl below 30 and an anticipated course longer than 10 days, after the initial 72 hours.
What to reconcile: Do not apply a reduced regimen universally. Retain the clinical-stability, duration, monitoring and specialist qualifiers.

Linezolid: qualified expert reduction versus label no-adjustment
UCSF describes an optional expert reduction for selected stable patients receiving a prolonged course, rather than a universal renal reduction. Its dialysis table remains separately stated.
What to reconcile: Document the source and qualifying clinical context; involve infectious diseases and consider drug-level monitoring. Do not apply the expert exception indiscriminately.

Renal dosing, restrictions and evidence limits
No renal dose adjustment is recommended because parent-drug concentrations are similar across impairment groups. The two primary metabolites accumulate as renal function declines; weigh the potential risks of that accumulation. Administer after hemodialysis. Information about peritoneal dialysis is unavailable. This label statement differs from selected expert strategies for prolonged treatment with monitoring; it does not establish that those strategies apply to every patient.
