Creatinine transport is not a blanket explanation for kidney injury
An isolated creatinine increase can reflect tubular transport, but diarrhea and dehydration can cause true injury. Severe-renal and dialysis exposure remain unknown.
What to reconcile: Use the clinical course and appropriate alternative renal markers; do not assume that every rise is benign.

Renal dosing, restrictions and evidence limits
The official label requires no renal adjustment at Cockcroft-Gault CrCl 30-89 mL/min. Severe impairment, ESRD and dialysis pharmacokinetics remain unknown.
Inhibition of tubular creatinine secretion can raise serum creatinine without affecting glomerular function. The label suggests alternatives such as BUN, cystatin C or a non-creatinine GFR assessment when needed. Severe diarrhea with dehydration is a separate safety concern; evaluate the patient's clinical course rather than assuming that every creatinine rise is benign.
