Do not merge label and higher-dose institutional dialysis protocols

The label includes CrCl exactly 30 in its >=30 band. UCSF prints >30 and <30, omitting exactly 30, and uses higher doses and different HD scheduling.

Adult regimen and renal adjustment

The displayed table uses 6 mg/kg IV daily for bacteremia, osteomyelitis and other severe infections, or 4 mg/kg daily for UTI/skin infection. At CrCl <30 mL/min it retains the selected dose but extends the interval to every 48 hours. HD follows the <30 interval, with administration after dialysis on treatment days.
