Renal dosing, restrictions and evidence limits

The JANTOVEN source does not require an automatic renal-failure dose reduction, but requires more frequent INR monitoring with compromised renal function. Dose and target INR remain individualized to response and indication. This is not a fixed-dose recommendation or a statement that kidney disease carries no bleeding risk.
