Labeled basic dosing
For edema, start 20-80 mg orally once. When needed, repeat after 6-8 hours or increase by 20-40 mg no sooner than 6-8 hours after the prior dose. The effective individual dose is subsequently given once or twice daily.

Renal information and dialysis
Dosing is response-guided, not a fixed CrCl percentage table. Anuria is a contraindication. Stop treatment if increasing azotemia and oliguria develop in severe progressive renal disease.
Follow fluid status, blood pressure, electrolytes and kidney function. Excess diuresis can cause dehydration and electrolyte depletion; severe renal dysfunction increases susceptibility to ototoxicity.
