Gout prophylaxis, flare treatment and interactions differ
The COLCRYS gout source has different dialysis instructions for prophylaxis and flares. Renal/hepatic impairment plus strong CYP3A4 or P-gp inhibition is a contraindication, not merely another dose-reduction row.
What to reconcile: Check interacting medicines and existing prophylaxis; do not substitute LODOCO cardiovascular instructions.

Renal guidance and important limits
For gout prophylaxis, the mirror calls for close monitoring without adjustment in mild/moderate impairment; severe impairment starts at 0.3 mg/day. Dialysis prophylaxis starts at 0.3 mg twice weekly. Acute-flare treatment in severe impairment is not repeated more often than every 2 weeks; dialysis flare treatment is a single 0.6 mg dose, also not repeated within 2 weeks. Consider alternatives for repeated courses. Flare treatment is not recommended in renally impaired patients already receiving COLCRYS prophylaxis. Renal or hepatic impairment with P-gp or strong CYP3A4 inhibitors is contraindicated. These are COLCRYS gout instructions, not LODOCO cardiovascular dosing or a familial Mediterranean fever protocol.
