Do not merge label and higher-dose institutional dialysis protocols

The label includes CrCl exactly 30 in its >=30 band. UCSF prints >30 and <30, omitting exactly 30, and uses higher doses and different HD scheduling.

What to reconcile: Keep source-specific dose, interval, indication, weight basis and modality together. The official threshold is explicit; seek institutional clarification rather than silently changing its headings.

Renal dosing, restrictions and evidence limits

The CUBICIN RF mirror gives 4 mg/kg every 24 hours for adult complicated skin/skin-structure infection and 6 mg/kg every 24 hours for adult S. aureus bloodstream infection when CrCl is at least 30 mL/min. Below 30, including HD and CAPD, the respective doses are given every 48 hours; administer after HD when possible. Pediatric renal dosing is not established. In renal impairment, renal function and CPK need assessment more frequently than weekly. These indication-specific label regimens are not the UCSF higher-dose protocol.
