Renal dosing, restrictions and evidence limits
Adults with CrCl below 15 mL/min, or receiving hemodialysis or peritoneal dialysis, receive 100 mg orally every six to eight hours in this label. At CrCl at least 15 mL/min, no renal reduction is required. This is an oral adult excerpt; it does not substitute for IV, neonatal prophylaxis or combination-product instructions.
