The handbook retains normal dosing above GFR 60, limits established treatment to 100 mg daily in the 45-60 range, and advises against initiation below eGFR 60 and use below 45. It advises avoidance during the listed replacement modalities and describes canagliflozin as not dialysed. These older cutoffs do not replace the current INVOKANA label.
