The institutional table lists 2 g every 6 hours for CrCl >120 mL/min and 2 g every 8 hours for 60-119. Its other non-dialysis doses and intermittent-HD 750 mg every 12 hours align with the selected official table; infusions run over 3 hours.

Its continuous-dialysis bands use effluent <=2, 2.1-3, 3.1-4 and >4 L/hour, paired respectively with 1.5 g every 12 hours, 2 g every 12 hours, 1.5 g every 8 hours and 2 g every 8 hours. Use is restricted to Infectious Diseases or Antimicrobial Stewardship.
