Fenofibrate renal starting strengths are product-specific
TRICOR uses its own starting strength. ANTARA, LIPOFEN and FENOGLIDE have different strengths; TRIGLIDE has a distinct lower-dose availability restriction.
What to reconcile: Confirm the dispensed formulation and its own renal instructions. Do not apply a generic milligram-equivalence rule.
Source-identity hold: title strength does not match capsule description
The separate RX fenofibrate-drug page is titled 40/120 mg but describes 50/150 mg capsules. No renal dosing from that mismatched page was imported in v4.1.

Renal dosing, restrictions and evidence limits
eGFR 30 to <60 mL/min/1.73 m2: start 48 mg orally once daily. Increase only after assessing renal function and triglyceride response. Use is contraindicated at eGFR <30 mL/min/1.73 m2, including ESRD and dialysis. Assess renal function before treatment and periodically during therapy. The starting strength belongs to this named product.
