EPZICOM: label restriction versus limited-evidence panel practice

The selected label does not recommend the fixed-dose combination below CrCl 30. NIH describes some panel members retaining full-dose lamivudine there but explicitly states that evidence is insufficient.

What to reconcile: Preserve the evidence qualification and obtain HIV-specialist or institutional adjudication. Do not present that exception as a new label-approved dose.
