EPZICOM: label restriction versus limited-evidence panel practice

The selected label does not recommend the fixed-dose combination below CrCl 30. NIH describes some panel members retaining full-dose lamivudine there but explicitly states that evidence is insufficient.

What to reconcile: Preserve the evidence qualification and obtain HIV-specialist or institutional adjudication. Do not present that exception as a new label-approved dose.

Renal dosing, restrictions and evidence limits

The selected RX labeling does not recommend EPZICOM at CrCl <30 mL/min. When lamivudine needs a renal reduction, use individual components rather than an invented fraction or interval for the fixed-dose tablet. This is distinct from the separately stored NIH discussion of selected clinicians continuing full-dose lamivudine despite limited evidence.
