Renal dosing, restrictions and evidence limits
In older adults, avoid baclofen when eGFR is below 60 because of encephalopathy risk. When unavoidable, use the lowest effective dose and monitor for CNS toxicity, including altered mental status. The source also identifies risk in chronic dialysis. This is not a numeric dose-reduction schedule.
