A renal infusion reduction is not a dialysis allowance

The infusion adjustment concerns low clearance in eligible adults. Dialysis dependency appears separately in the contraindications.

What to reconcile: Check dialysis status and bleeding contraindications before applying the infusion reduction. Retain bolus, duration and co-antithrombotic rules from the same protocol.

Renal dosing, restrictions and evidence limits

At CrCl below 50 mL/min, reduce the infusion to 1 mcg/kg/min. The indication-specific 180 mcg/kg bolus instructions are not reduced by this renal rule; PCI includes its specified second bolus. Dialysis dependency is a contraindication, not an invitation to extend the low-clearance infusion band. Bleeding risk, other antithrombotics, platelet counts and full treatment-duration instructions require separate review.
