Renal-band endpoints differ between the label mirror and UCSF
Both tables use indexed eGFR, but the label assigns exactly 50 to its full-dose band, whereas UCSF prints >50 and 30-50. UCSF also prints overlapping bands at exactly 30. The dialysis and indication scopes are separately stated.
What to reconcile: Do not silently round an estimate, substitute unindexed CrCl, average doses, or combine one source's band with another interval. Clarify boundary cases with the applicable label and institutional antimicrobial team.

For adult complicated UTI including pyelonephritis, infuse each dose over 3 hours; label treatment duration is up to 14 days.

Give renal-adjusted doses after HD. The label uses indexed MDRD eGFR, not unindexed Cockcroft-Gault CrCl. Do not apply a meropenem-alone regimen to this combination.
