Renal dosing, restrictions and evidence limits

No clinically important pharmacokinetic difference was reported with mild or moderate impairment, described as eGFR 30 to <90 mL/min. Effects in severe impairment, eGFR 15 to <30, and end-stage disease, eGFR <15, are unknown. These are pharmacokinetic findings, not a stand-alone no-adjustment recommendation or dialysis schedule.
