Dosing / renal protocol and limits
Short-infusion IV dosing by CrCl: >50, 2 g every 8 hours; 31-50, 2 g every 12 hours; 15-30, 2 g daily; <15, 1 g daily.
IHD: 1 g initially and following each dialysis. CVVHD: 2 g every 12 hours.
A separate specialist-selected extended-infusion strategy uses 2 g over 30 minutes as loading, then maintenance beginning 4 hours later. Maintenance infusions last 4 hours: 2 g every 8 hours above CrCl 50 or every 12 hours at 31-50. Below 31, use short infusion. CVVHD extended infusion also requires ID/pharmacy selection.
