The published columns omit exactly 5 mL/min; that boundary is not repaired by assumption.
IHD: 0.94 g initially, then each evening. For minimal residual kidney function and less severe infection, an alternative is 0.94 g every 48 hours in the evening. CVVHD: 2.5 g every 8 hours. All these institutional infusions last 3 hours; retain separately from product-label infusion instructions.
