Dosing / renal protocol and limits

The standard adult systemic regimen is 500 mg IV/orally every 8 hours when CrCl >10 mL/min and every 12 hours below 10. Exactly 10 is omitted in the displayed bands.

For bacterial vaginosis, vaginal trichomoniasis or intra-abdominal infection other than C. difficile, 500 mg every 12 hours is retained in both renal columns. During either IHD or CVVHD, the standard regimen is 500 mg every 8 hours; the listed indication-specific every-12-hour regimens remain every 12 hours. Do not infer dialysis dosing from the non-dialysis low-clearance row.
