Dosing / renal protocol and limits
After a 2 g IV load, standard-infection CVVHD dosing is 1 g every 8 hours at 1 L/hour; 1 g every 6 hours at 2 L/hour; and 1 g every 6 hours or 2 g every 8 hours at 3-4 L/hour. The neutropenic-fever row instead lists 2 g every 12 hours at 1-2 L/hour and 2 g every 8 hours at 3-4 L/hour. Reassess when therapy stops.
