Select the usual regimen before the renal row
The 750, 500 and 250 mg renal paths differ. The 250 mg row has a specific uncomplicated-UTI exception and lacks dialysis adjustment information.

For the 250 mg/day regimen, no adjustment at 20-49; at 10-19 use 250 mg every 48 hours except uncomplicated UTI, for which the source says no adjustment. No HD/CAPD adjustment information is supplied for that 250 mg row. At CrCl at least 50, the renal table requires no adjustment. Fluoroquinolone boxed warnings and indication restrictions remain separate selection requirements.
