The label does not require routine renal reduction. UCSF describes a selected strategy for stable patients with CrCl below 30 and an anticipated course longer than 10 days, after the initial 72 hours.

Renal dosing, restrictions and evidence limits

Parent linezolid pharmacokinetics do not require renal dose adjustment, but inactive metabolites accumulate as renal function worsens; weigh the uncertain clinical significance against potential risks. The label directs administration after hemodialysis and reports approximately 30% removal in the described three-hour session. Peritoneal-dialysis pharmacokinetics are unavailable. This no-adjustment statement does not eliminate toxicity surveillance.
