The moderate-renal mCRPC dose disagreement is still present
The current official label states 0.35 mg daily with enzalutamide at CrCl 30-59; the retrieved RX mirror still states 0.5 mg daily for that setting. Breast-cancer monotherapy is a separate regimen.
What to reconcile: Keep the conflict flagged and prioritize the exact official product label; do not average the doses or transfer the monotherapy schedule.

Renal dosing, restrictions and evidence limits
For mCRPC with enzalutamide, CrCl 30-59 mL/min requires 0.35 mg once daily; CrCl 15-29 requires 0.25 mg once daily. Breast-cancer monotherapy uses 0.75 mg daily at 30-59 and 0.5 mg daily at 15-29. These are separate indication-specific renal regimens.
Hemodialysis has not been studied. Review hematologic toxicity and interacting drugs separately. This dated official renal paragraph does not resolve the mirror's different moderate-renal mCRPC amount by averaging; the mirror remains flagged.
