The RX table prints >30 to 59 and >15 to 29; exactly 30 is not clearly assigned. The hemodialysis supplement also belongs with a selected maintenance regimen.

Renal dosing, restrictions and evidence limits

The immediate-release table lists total daily doses of 900-3600 mg in three doses at CrCl >=60; 400-1400 mg in two doses in its printed >30 to 59 band; 200-700 mg once daily in its printed >15 to 29 band; and 100-300 mg once daily at 15. Below 15, reduce the daily dose proportionally. The printed bands do not clearly assign every boundary, notably exactly 30; do not silently round into a regimen. Hemodialysis requires a maintenance regimen plus a supplement after each four-hour treatment; use the complete paired source table rather than selecting an unpaired supplement. Renal dosing below age 12 was not studied.
