Renal dosing, restrictions and evidence limits
Discontinue if increasing azotemia and oliguria develop during severe progressive renal disease. Severe renal impairment, rapid parenteral administration and other ototoxic exposures can increase hearing-toxicity risk. Excess diuresis can cause volume and electrolyte depletion. These warning statements do not supply a universal dose reduction or escalation rule.
