Renal dosing, restrictions and evidence limits
The July 2020 manufacturer PDF contraindicates eGFR <40 mL/min/1.73 m2 and reduces the starting dose by 50% at 40-60. Evaluate renal glomerular and tubular function before starting or increasing therapy. Monitor eGFR and tubular toxicity weekly in the first month after initiation or modification and at least monthly thereafter; renal-risk patients need more frequent assessment. Pediatric volume-depleting illness requires interruption and reassessment before resumption. On-treatment dose modifications remain indication- and age-specific. The dated PDF is not proof that a full current-label reconciliation has been completed.
