Dosing / renal protocol and limits

No routine renal dose reduction is listed, including hemodialysis. Adult non-CNS mycobacterial infection: 600 mg IV or orally daily. Prosthetic-device infection: 300 mg every 12 hours or 600 mg daily. Endocarditis: 300 mg every 8 hours. Choose an indication-specific combination regimen and review major drug interactions; these entries do not authorize rifampin monotherapy.
