IV vehicle warning versus qualified institutional short-course use
VFEND favors oral therapy below its renal threshold unless an individualized benefit-risk assessment justifies IV use. UCSF acknowledges that warning and discusses limited evidence for short IV courses in selected patients. The guidance differs in emphasis, not through removal of the vehicle warning.
What to reconcile: Check the exact IV product and excipient, oral feasibility, intended duration, creatinine trend, interactions and drug-level monitoring. Neither source supports ignoring renal deterioration or automatically reducing the oral dose.

Renal dosing, restrictions and evidence limits
Oral voriconazole requires no renal adjustment in mild through severe impairment. Below CrCl 50, the IV vehicle SBECD accumulates; prefer oral treatment unless an individualized benefit-risk assessment justifies IV VFEND. Follow serum creatinine closely and consider switching to oral therapy if it rises. Four-hour hemodialysis does not remove enough voriconazole to require a dose adjustment. Pediatric renal adjustments are not established. This formulation-specific vehicle issue does not prove all IV products have the same excipient, and it is not a reason to reduce an oral dose automatically.
