Vancomycin - Predicted Trough Value - Level drawn early
This program can be used to determine if the current regimen is appropriate based on an early (supratherapeutic) vancomycin level which is used along with an estimated elimination rate constant to determine the level just before the next dose.
(Note: if a random supratherapeutic vancomcyin level is drawn and you need to determine when to give the next dose based on a desired target trough level -
use the following program
. Overview: An estimated elimination rate constant is generated from the creatinine clearance which is then used to determine the timing of the next dose based on the target trough concentration.)
Estimated creatinine clearance
:
(mL/min)
Current trough (early supratherapeutic value)
:
mg/ml
Number of hours trough was drawn PRIOR to next dose
:
hrs.
(Note: may use decimals e.g. 3.5 hrs. This time will be used to calculate the estimated trough value just before the next dose is due.)
