Usual Dosing (Adults)

Ventricular arrhythmias:
Initial: 80 mg orally twice daily. Dose may be increased gradually to 240-320 mg/day - allow 3 days between dosing increments. Usual range: Most patients respond to 160-320 mg/day in 2-3 divided doses.

Renal Dosing

dialysis Ventricular arrhythmias (Betapace ®, Sorine ®):
[CRCL >60 ml/min]: Give q12h.
[30-60]: Give q24h.
[10-30]: Give q36-48 hours.
[<10]: Individualize dose.

Atrial fibrillation/flutter (Betapace AF®):
[CRCL >60 ml/min]: Give q12h.
[40-60]: Give q24h.
[<40]: Use is contraindicated.

Hemodialysis

dialysis Administer dose post-dialysis or administer supplemental 80 mg dose. Peritoneal dialysis does not remove sotalol - supplemental dose is not necessary.
