Usual Dosing (Adults)

HCL salt: 65 mg q3-4 hours prn pain (maximum: 390 mg/day).

Napsylate salt: 100 mg q4h prn pain (maximum: 600 mg/day).

Renal Dosing

dialysis [> 10 ml/min]: No change. 

[<10]: Avoid.   (Serum concentrations of propoxyphene may be increased to toxic levels.)

Hemodialysis

dialysis Avoid.
