Usual Dosing (Adults)
250 – 500 mg po q12h.

Renal Dosing
dialysis [CRCL >30 ]: No changes. 

[<29 ml/min]: 50% of usual dosage q12-24h.

Hemodialysis
dialysis 50% of usual dosage q12-24h. (Administer after the completion of hemodialysis on dialysis days).
