Operating instructions    TOP
First step: Under 'Converting from' select the first opiate you want converted. Under 'Converting to' select the opiate you want to convert to. Second step: If you click the 'Modify dosage table' button you will be able to see the equianalgesic conversion factors. You may modify these if necessary. Third step: (optional) Using this screen you can select additional opiates to convert in addition to what was selected under 'Converting from' on the first screen. This valuable screen is perfect for patients on multiple opiates and you would like to convert the patient to a single opiate. To advance to the next screen hit the 'Calculate button'

Forth step: This is the final results screen.  The results of the calculator are listed at the top.  The 'Methadone Conversions' option utilizes the calculated morphine equivalents to create a methadone dosing schedule. Fentanyl equivalent dosages. See previous screen for accessing this table. The table below lists the conversions used to determine equivalent fentanyl patch requirements based on the manufacturer's guidelines. Methadone conversion algorithm - User-adjusted Hybrid method:  With this method, you will be able to modify the conversion ratios used to convert from other opiates to methadone. Several dosage ranges (e.g. 0-99 mg, 100-299mg, ...) and the corresponding ratio used to convert from morphine equivalents (mg)  to methadone are listed. If you disagree with the default settings, simply press the dashed box around the numeric range (e.g. 500-990), and you will see a drop down list of choices. Simply select the ratio that you feel is the most appropriate.

Hints for using the program: (1) Whenever you see a dashed box around an object, this means that there is additional information in a secondary window when the object is clicked on.  (2) The drop down menus on the third screen seen below are used only if you are converting more than one opiate at a time. For example if a patient is on hydromorphone IV and oral, as well as IV morphine you could convert all 3 of these to a single opiate (e.g. oral morphine). In this example you would select oral morphine under 'converting to' on the first screen and use the other drop down menus to select the other opiates: hydromorphone oral, hydromorphone IV and morphine IV.
