Usual Dosing (Adults)

HIV: 150mg po q12h or 300mg po qd.
Hepatitis B (Epivir-HBV ®): 100mg po qd.

Combivir ®: [150mg lamivudine/ 300mg AZT]: 1 tablet bid.

Renal Dosing

dialysis Lamivudine-Epivir ®:
HIV:
[CRCL > 50]: No changes
[30-49]: 150mg po qd.
[15-29]: 150mg po x 1, then 100mg po qd.
[5-14]: 150mg x 1, then 50mg qd.
[<5]: 50mg x 1, then 25mg qd.

Hepatitis B:
[CRCL > 50]: No changes
[30-49]: 100mg po x 1, then 50mg po qd.
[15-29]: 100mg po x 1, then 25mg po qd.
[5-14]: 35mg po x 1, then 15mg po qd.
[<5]: 35mg po x 1, then 10mg po qd.

Lamivudine/Zidovudine - Combivir ®:
[CRCL >50 ml/min]:  No changes.
[<50]:Give each agent separately and adjust based on respective renal dosing guidelines.

Hemodialysis

dialysis Lamivudine- Epivir ®:
HIV: 50mg x 1, then 25-50mg qd.
Hepatitis B: 35mg po x 1, then 10mg po qd.

Lamivudine/Zidovudine - Combivir ®:
Give each agent separately and adjust based on respective renal dosing guidelines.
