Usual Dosing (Adults)

Treatment: 800mg qhs, or 300mg PO/IV q6h, or 400mg q12h.
Maintenance: 400 mg qhs.
Esophagitis/reflux: 800mg bid or 400mg po q6h.
Maximum dose/day: 2400 mg.

Renal Dosing

dialysis [>50 ml/min]: No changes.
[10-50]: Give 75% of usual dose at usual interval.
[<10 ml/min]: Give 50% of usual dose at usual interval.

Alternative:
PO:
[>30 ml/min]: no change
[15-30]: usual dose q8h
[<15]: usual dose q12h
IV:
[>50 ml/min]: 300mg q6h
[30-50]: 300mg q8h
[10-29]: 300mg q12h
[<10]: 300mg q24h

Hemodialysis

dialysis [<10]: Give 50% of usual dose at usual interval. Some sources recommend giving 300mg 24h.  Doses should be scheduled after dialysis on dialysis days.
