Usual Dosing (Adults)
< 50 kg: 20 mg/kg twice daily (Max: 2400 mg/day)
> 50 kg: 1200 mg twice daily
Dosage must be adjusted when administered in combination therapy.

Renal Dosing
dialysis No adjustment in renal failure.

*Avoid oral solution in renal failure due to possible accumulation of propylene glycol

Hemodialysis
