Amikacin - Amikin ® - Renal dosing

Usual Dosing (Adults)

Susceptible infection: 7.5 mg/kg IV q8 to 12h.

Renal Dosing

[CRCL > 60 ml/min]: Give q8h (follow levels).

[40-60]: Give q12h (follow levels).

[20-40]: Give q24h (follow levels).

[10-20]: Give q48h (follow levels).

[< 10 ml/min]: Give q48-72h (follow levels).

Hemodialysis

Hemodialysis: Administer dose post-dialysis or administer 2/3 normal dose as a supplemental dose post-dialysis and follow levels.
