Allopurinol - Zyloprim ® - Renal dosing

Usual Dosing (Adults)

Gout: initially 100mg qd. Increase by 100mg/day q7days, titrate to desired uric acid level. Usual range: (Mild gout): 200-300mg/day. Moderate-severe: 400-600mg/day.

Maximum dose/day: 800mg

Renal Dosing

dialysis Oral:
[>80 ml/min]:  Usual dose
[60 - 80]: 200-250 mg daily
[40 - 60]: 150 - 200mg daily
[20 - 40]: 100 - 150mg daily
[10 - 20]: 100mg q24
[<10 ml/min]: 100 mg q24-48h

[>50 ml/min]: No changes
[20-50]: 100-300mg q24h.
[10-20]: 100-200mg q24h.
[<10]: 100mg q24-48h.

[10 - 20 ml/min]: Administer 200 mg/day.
[3 - 10 ml/min]: Administer 100 mg/day.
[<3 ml/min]:  Administer 100 mg/day at extended intervals.


dialysis 100mg q24-48h (give dose after dialysis on dialysis days.)
